Telemedicine Program - Working in collaboration with the BAFHT physicians and the Telemedicine Coordinator, the Telemedicine Nurse Practitioner provides primary care services to individuals who require a primary care provider and/or connection with CMHA-GB * program services are available to people aged 18 years or older and who have either a serious mental illness or substance use disorder (excluding tobacco as a primary addiction)
