Case managers from the Home and Community Care Support Services South West Region available on site

Ambulatory Care - Outpatient specialist clinics * diagnoses and treatments for various medical conditions

Complex Care Program - Continuing medical management and a range of interdisciplinary, diagnostic, therapeutic and technological services * assessment, restoration, reactivation, palliative care and continuing care placement

ConnectCare - Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response

Mental Health Care Building - Inpatient and outpatient mental health services at the tertiary and long-term level

Rehabilitation Programs - Inpatient and outpatient rehabilitation programs for people recovering from spinal cord injury, acquired brain injury, trauma, stroke, amputations, and neurological conditions

Specialized Geriatric Services - Specialized care of older persons * evaluation and treating of functional, medical, and psycho-social aspects of illness and disability
