Service Description: Peer support groups, education, and training for family members, service providers, and caregivers * sessions may include information such as advanced directives and safety issues

Services include:
  • Southwestern Ontario Regional Geriatric Program
  • Southwestern Ontario Regional Psychogeriatric Program - Enhances the care of older adults who are challenged with mental health problems such as depression, psychotic disorders and dementia * program consists of a geriatric psychiatrist and clinical nurse specialist with expertise in mental health issues * services provided by the team include clinical consultation, education, and community development
  • Geriatric Mental Health Program - Psychiatry consultation, evaluation and medication services for older adults
  • Discharge Liaison Team - Support for inpatients with complex, yet stable mental health needs who no longer require specialized hospitalization as they transition from hospital to a long term care home
  • Third Age Outreach
  • Geriatric Resource Nurse Outreach Team - Seven RNs specializing in care of frail elderly * provide assessments and recommendations for patients in Huron, Perth, Grey, Bruce, Elgin and Oxford Counties
