Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * Wellness for Seniors programs include exercise, health screenings, meals and foot care * referral to other community supports referral to other community services
 
Accessible Tub and Shower Room - Whirlpool tub and electronic lift available for people who require accessibility supports. There is a fee of $15/hour and client is responsible for own support person if required. Hours are by appointment only.
 
Blood Pressure Program - As part of our Wellness Program, our Blood Pressure Program's goal is to promote cardiovascular health, healthy lifestyles, personal empowerment, and regular blood pressure monitoring for participants.
 
Meals on Wheels - Hot meals delivered Mon-Fri 4:30pm-5:30pm to participants living within the community of Zurich * meals include a salad, entree, and dessert * special diet accommodations diabetes-friendly options are available upon request
 
Wellness for Seniors - Wellness programs, including education, exercise and falls prevention and health screening for adults who are 55 years of age and older
Fees: Community Outreach fees are dependent on program
Application: No referral required for Community Outreach * Participants do need to complete registration by calling Central Intake at 1-844-482-7800
