South West Community Care Access Centre
Call for Application to Become a Member

Are you motivated to be involved with and contribute to an organization with an exciting future? The South West Community Care Access Centre serves 60,000 people each year, across a vast region from Tobermory in the north to Long Point and Port Glasgow in the south. Our role is to get people the home and community care they need to stay well, heal at home and stay safely in their homes longer. We also help people transition through the system and to other living arrangements. We do it by working in partnership with patients, families, providers, community organizations, and others.

Our vision is Outstanding Care – every person, every day.

As a member of the Board of Directors you will have an opportunity to shape the future of home care in our region, working with dedicated and engaged Board colleagues and senior leaders of the South West CCAC.

Time commitments include attending Board meetings, generally held eight times a year from 11:00 am to 3:00 pm, and regular Committee involvement. Travel requirements include attending meetings at the Head Office in London and, throughout the year, at our CCAC office locations across the South West geographic area. The initial term of office is for three years.

To be eligible, you must:

  • Be 18 years of age or older
  • Live in the South West CCAC geographic area

We strive to ensure a balance of skills among our Board Members to provide leadership and strategic direction to the CCAC. We welcome applicants to appoint up to three new Directors who are systems thinkers with an understanding of health care issues, and who are seasoned governors familiar with generative and collaborative governance, including an individual also with financial expertise.

For more information about the South West CCAC, please visit our website at www.healthcareathome.ca/southwest.

For an Expression of Interest package, please contact Cate Patchett, Corporate Liaison, at 519 641 5482 or cate.patchett@sw.ccac-ont.ca. Applications will be accepted until Thursday, April 30, 2015.
