Service Description:
|
Access to health care support at home, at school, or in the community * care coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care * care is provided in community settings such as the home, school, residential facility or Ontario Health at Home community clinic * referral to other community services when appropriate
Services available to eligible patients may include:
- nursing
- nurse practitioner
- personal support
- physiotherapy or occupational therapy
- speech and language therapy
- social work
- nutrition and food counselling (dietitians)
- end-of-life care
- medical supplies and equipment (patient-specific supplies for Ontario Health at Home Community care, such as catheters, ostomy bags, or wound care supplies)
- access to long-term care homes
- access to supported living (adult day programs, supportive housing, retirement homes, and assisted living)
- health support in the school
- specialized services for people living with specific health conditions
- referral to other community supports

- Nursing services for ambulatory patients at clinics located in the community

Ontario Health atHome - Health Care Connect
- Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector employed by Ontario Health atHome* Care Connectors work with family doctors and nurse practitioners who are accepting new patients

Ontario Health atHome - Information and Referral
- Provides information on health care services available in the community or in-home * assesses needs of new clients * Connects clients with health care resources

Ontario Health atHome - Mental Health and Addictions School Program
- Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate
|

Ontario Health atHome - School Health Support Services
- Special health services for children who require medical or rehabilitative assistance to attend school, participate in school routines and receive instruction * services are provided within public, private or home-based schools
|

Ontario Health atHome - Telehomecare Program
- 6 month telephone coaching, support and monitoring program patients with mild to moderate Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) * equipment provided (blood pressure, weigh scale, pulse oximeter and tablet) to report on health daily * weekly discussions with telehomecare nurse
