Service Description:
|
Provides in-person, telephone and virtual services to youth, adults, and older adults with addiction and mental health needs who reside in Huron and Perth counties
- addiction counselling for youth and adults
- mental health case management
- peer support
- court support
- couselling for problem gambling/behavioural change
- supportive housing
- community withdrawal management

Adult Substance Use Program
- Free, practical and confidential services to anyone in
Huron and Perth Counties who have concerns about their own or someone
else’s alcohol use, or drug use
|

Case Management
- Works with individuals experiencing symptoms of mental
illness who require support to live and work in the community * case
managers provide on-going support as needed in order to stabilize,
achieve goals, and improve quality of life * Case Management services are
tailored to the individual, based on their identified goals for
support * participation in the Case Management Program is a prerequisite
to obtain CMHA Huron Perth supportive housing

Community Homes for Opportunity
- A congregate care, supportive housing program, providing
24-hour, on-site support to people diagnosed with a serious mental illness * residents receive support with medication management, life-skills coaching, budgeting, support and advocacy while attending medical appointments and community integration
|
|
Court Diversion Program and Court Support Services
- Partners with the criminal justice system and the mental health system to provide direct service to people who have a serious mental illness, developmental challenges, or brain injury who have been charged with a minor offence * program promotes treatment and rehabilitation to prevent recidivism

Crossing Bridges - Virtual Youth Counselling
- A confidential, virtual safe space for young people to:

Family Support Services
- Provides family members, caregivers, and friends with tools to help in the recovery process of a loved one * participants will learn about mental illness, addiction, coping strategies and community support for both themselves and their loved one struggling with their mental health or addiction * services include group and one on one education sessions, peer support, and skills coaching

Peer Outreach Services
- Peer Outreach Workers use their own lived
experience with mental health and addiction to walk alongside people in their journey of recovery and wellness * provide
supportive listening and assist with navigating the mental health,
addiction, and health care systems
|

Phoenix Peer Support Centre
- Offers a variety of groups and drop-in peer support services for both youth and adults * staff and volunteers within this program have their own lived experiences with mental health and addiction recovery, and offer supportive listening and facilitation in connecting to other resources
as needed

|
Problem Gambling and Behaviour
- Free, practical and confidential services to anyone in Huron and Perth counties who may be struggling with issues related to problematic behaviours
