Service Description: Living alternatives to hospitalization in a home-like setting with staff support
  • individual and/or family units
  • shared living with low to moderate support
  • shared living with high support
  • youth (16-24) transitional shared living and individual units
  • women only – individual units
  • addictions supportive individual units
  • justice units
 
Eating Disorders Residence - Voluntary 8-bed residence for adults with an eating disorder * specialized supports and programs for adults with an eating disorder diagnosis who require 24 hour support to interrupt symptoms
 
Long Stay Residential Treatment Facility (RTF) - Residential program for people with a serious mental illness who may also have physical health concerns and need access to 24-hour supports with clinical interventions for an extended period of time
 
Shared Living - One women-only home and one men-only home with 4 beds each * low support 10am-5pm
 
Supportive Living Apartments - Independent living with daily on-site staff support * bachelor, one bedroom and two bedroom units available * each building has a lounge, kitchen, office, and private meeting area
 
Transitional Residential Treatment Facility (RTF) - Short stay residential program for people with a serious mental illness who need access to 24-hour supports with clinical interventions
 
Transitional Shared Living - 3 24-hour moderate support homes with a minimum of 8 hours of support per day
Fees: Subsidized monthly rent
Application: Referral from a mental health professional or visit website * referral specific to site
Eligibility - Population(s) Served: Adults and youth 16 years and older with persistent mental illness who are characterized as complex with a history of lengthy and/or multiple hospital stays * have rehabilitation potential and readiness as well as a desire for improved mental health
