Service Description:

Education and support in managing or preventing responsive behaviours for older adults and their caregivers that may be associated with dementia, complex mental health, substance use, addiction, or neurological conditions * responsive behaviours include aggression, wandering, physical resistance or agitation
Links to community resources including:- specialized geriatric medical and psychiatric services
- geriatric mental health outreach teams
- community support services
- geriatric emergency management nursing
- inpatient geriatric assessment units
- primary care based memory clinics
- Adult Day Programs
- Alzheimer Society education, counseling and support programs
- long-term care and retirement homes
The South West BSO Operations Team collaborates with the BSO Provincial Coordinating Office, Ontario Health West, Sub-regional Mobile Teams, Alzheimer’s Societies, Adult Day Programs, Long Term Care Homes to realize the BSO Mandate.
The Operations Team:- Provides backbone support for BSO education in the South West.
- Collects, Interprets and Reports BSO Data to the PCO quarterly
- Hosts regional engagement opportunities
- Utilizes strategic quality improvement measures to support regional initiatives and advocacy
