Service Description: Interdisciplinary team that provides services and supports for older adults, their families and caregivers, who are having challenging responsive behaviours associated with dementia, mental illness, addictions and other neurological conditions * team includes nurses, social workers, occupational therapists, recreational therapists, and psychologists

Services include:
  • assessment started within 1 week of referral with higher risk individuals started sooner
  • behavioural and biopsychosocial assessments, aimed to identify the unmet needs influencing responsive behaviours
  • completion of standardized screening tools
  • recommendations to promote effective communication between the person with responsive behaviours and care providers
  • non-pharmaceutical recommendations to decrease responsive behaviours
  • support for caregivers in implementing recommendations and evaluating the impact
  • outreach support (case managers visit clients in homes, retirement and long term care homes, hospital or other community support settings, such as Adult Day Programs)
  • formal and informal education, in-service, and workshops on the topic of Responsive Behaviours
  • brief follow-up

    Eligibility - Population(s) Served: Adults aged 65 +, with responsive behaviours of risk, related to dementia, mental health, addictions, and/or related neurological conditions., where responsive behaviours are placing the client or others at high to moderate risk for harm if preventative measures are not put in place
