Service Description: Interdisciplinary team that provides services and supports for older adults, their families and caregivers, who are coping with or at risk for developing responsive behaviours associated with dementia, mental illness, addictions and other neurological conditions * team includes nurses, social workers, occupational therapists, recreational therapists, and psychologists

Services include:
  • assessment atarted within 72 business hours of referral
  • behavioural and biopsychosocial assessments, aimed to identify the unmet needs influencing responsive behaviours
  • completion of standardized screening tools
  • recommendations to promote effective communication strategies between the person with responsive behaviours and care providers
  • non-pharmaceutical recommendations to decrease responsive behaviours
  • support for caregivers in implementing recommendations and evaluating the impact of recommendations
  • outreach support (case managers visit clients in homes, retirement and long term care homes, hospital or other community support settings, such as Adult Day Programs)
  • formal and informal education, in-service, and workshops on the topic of Responsive Behaviours
  • brief follow-up

    Eligibility - Population(s) Served: Adults aged 65 +, with responsive behaviours of risk, related to dementia, mental health, addictions, and/or related neurological conditions., where responsive behaviours are placing the client or others at high to moderate risk for harm if preventative measures are not put in place
