Service Description:

Supportive housing program located in private homes, with admission, programs and supports provided by a community mental health team
Provides housing, meals and support services in a home setting for people with serious mental illness * recovery-based approach * provides access to support services in the home and in the community * opportunities to enhance quality of life, personal growth and development, and improve life skills through various activities and programs

Eligibility - Population(s) Served:

Adult residents of Ontario with serious mental illness (may include addictions) who need some level of support on a 24 hour basis
