Service Description: Short stay residential program for people with a serious mental illness who need access to 24-hour supports with clinical interventions

Opportunity for persons who have been hospitalized in a specialized psychiatric facility, or who are at risk of hospitalization, to move toward reintegration into the community through a home-like environment and interactive community setting

Length of stay is between 1 to 3 months, depending on treatment needs and goals
Fees: Subsidized monthly rent
Application: Referral from a mental health professional or visit website
Eligibility - Population(s) Served: Adults and youth 16 years and older who require 24-hour support with intermittent clinical interventions * need to be able to care for own physical needs, i.e. hygiene, eating, dressing
